If there has been damage to buildings or collections at a National Trust Historic Site, the Site staff should directly notify our insurance brokers Maury, Donnelly, & Parr, National Trust Insurance Services, as soon as possible after the incident (requested within 48 hours) by submitting this NTHP Damage/ Incident Report Form. The point-person in the office for claims at our Sites is Territa Sullivan, who can be reached on tsullivan@mdpins.com , telephone 410-547-3267 or fax 443-524-0959. Please email or fax the claim form directly to Territa Sullivan (e-mail is preferred), and also send a copy to:

- Katherine Malone-France, Director of Outreach, Education and Support, Historic Sites, Katherine_Malone-France@nthp.org and
- Lyn Moriarity, Administrative Director, Historic Sites, Lyn_Moriarity@nthp.org.
Maury, Donnelly & Parr will be responsible for following up with the Sites’ staff about the claim and completing subsequent reporting procedures. Leslie Kamrad, Katherine Malone-France and Lyn Moriarity will report claims internally to the appropriate parties within the National Trust for Historic Preservation.
Sites staff are always welcome to contact Territa Sullivan directly to inquire about the status of a damage report or claim.
